When your baby’s first teeth erupt in the mouth, it’s a big step in their development. Unfortunately, you may not have much opportunity to celebrate — you’re too busy tending to your infant whose experience is anything but pleasant.
Commonly known as teething, the eruption process usually begins between six and nine months of age, although some children may begin as early as three months or as late as twelve. Not all teeth come in at the same time: it usually begins with the two lower front teeth, then the two upper front teeth, followed by the molars and then the canines (eye teeth). By age three, most children have all twenty of their primary teeth.
Each child’s teething experience is different and may vary in length of time and intensity. The usual signs are heightened irritability, biting and gnawing accompanied by gum swelling, ear rubbing, drooling and sometimes facial rashes. Babies also may have disturbed sleeping patterns and a decreased appetite. Occasionally, this discomfort can be intense.
There are some things you can do to ease this discomfort. Provide your baby a clean, chilled (not frozen) rubber teething ring, chilled pacifier or wet washcloth to gnaw on. Cold foods, like popsicles for older children can also be soothing, though you should limit sugary foods to lower the risk of tooth decay. You can also finger massage swollen gums to counteract the pressure coming from the erupting tooth, or administer pain relievers like baby acetaminophen or ibuprofen. You can use products with Benzocaine®, a numbing agent, for children two years or older — but you should never use alcohol for children of any age for inflamed gums.
Be sure to also set up a Year One dental examination around their first birthday. This is an important first step in your child’s long-term dental care, and a good opportunity to check their teething progress. Teeth are composed of layers of different types of tissue. The main inner layers — the pulp and dentin — help the teeth respond and adapt to external forces. But they’re vulnerable to decay and quite sensitive to environmental extremes. They are protected from all these by a coating of enamel, made of the hardest material found in the human body.
But while enamel is strong, it’s not invincible — it can soften and dissolve (de-mineralize) if the mouth environment becomes too acidic. While de-mineralization occurs normally whenever the mouth becomes too acidic after eating or drinking, saliva helps neutralize the acid (buffering); in fact, saliva can restore to the enamel some of the calcium and other minerals it has lost (a process called re-mineralization).
If the acidic level remains too high for too long it can overwhelm saliva’s buffering ability and cause permanent mineral loss to the enamel. This erosion leaves teeth more susceptible to decay and disease and could lead to tooth loss. With this in mind, here’s some ways you can help preserve your enamel:
- Wait about thirty to sixty minutes after eating or drinking to brush your teeth. Counterintuitive as this may sound, it takes about thirty minutes for saliva to restore a normal pH level and re-mineralize the enamel. If you brush within this window of time, you could brush away some of the softened minerals from the enamel.
- Only brush twice a day. Over-brushing causes undue enamel wear, as well as contributing to receding gums.
- Take advantage of less acidic or mineral-rich beverages. Drink milk or water most of the time, rather than juices, sodas or sports drinks. The calcium in milk or as an additive to acidic beverages aids in buffering and re-mineralization.
- Use a straw for acidic beverages. With a straw your teeth avoid direct contact with most of the beverage’s acid, a key factor in de-mineralization.
- Avoid eating just before bed. Saliva flow decreases significantly when we sleep. If you eat right before bed, there may be less saliva available for buffering and re-mineralization.
Following these tips, along with effective oral hygiene, will go a long way in protecting your teeth’s enamel coating — and preserving your teeth in the long run.

Diabetes and periodontal (gum) disease are two types of inflammatory conditions that have more in common than was once thought. There is strong evidence to show that each of these diseases is a risk factor for the development and growth of the other. Studies have also found that treating one condition successfully may have a positive impact on the treatment of the other.
From the Greek meaning “to pass through the urine,” diabetes mellitus causes an abnormal rise in blood glucose level that can't be adequately controlled by insulin, the body's primary hormone for that task. Either the pancreas can't produce an adequate supply of insulin (as with Type 1 diabetes) or there is resistance to the hormone's effects (as with Type 2 and gestational/pregnancy diabetes). If you are a diabetic patient, you face many difficult issues with your health: your body develops an altered response to inflammation that may severely inhibit wound healing. You also may become more prone to chronic cardiovascular disease.
Periodontal (gum) disease describes a group of diseases caused by dental plaque, a whitish film that contains infection-causing bacteria. As infection rises within the gum tissues, the auto-immune system of the body responds to this threat and inflammation results. If the person is also a diabetic, this response may be impaired and may have a direct effect on how severe the periodontal disease progresses.
Periodontal disease can also affect your blood glucose level, if you are a diabetic. A number of studies have demonstrated that diabetic patients who have improved control of their periodontal disease through better oral hygiene and dental treatments have shown improvement in their blood sugar levels. There's even some evidence that effective periodontal treatment that reduces inflammation may improve the body's sensitivity to insulin. Likewise, bringing diabetes under control with supplemental insulin or positive lifestyle changes can help lessen the likelihood and severity of periodontal disease.
To sum it up, if you have been diagnosed with some form of diabetes, taking care of your teeth and gum tissues can have a positive impact on your diabetes. Likewise, making healthy changes in your lifestyle to bring your diabetes under control can reduce your risk for periodontal disease.

Tooth decay can be relentless: left untreated it can work its way into a tooth’s inner core — the pulp chamber or better known as the root canal. Once this occurs, the best course of action to save the tooth may be a root canal treatment to clean out the diseased pulp (nerve) and seal the canal from further decay.
So, what signs and symptoms might you encounter if decay has invaded a tooth’s root canal? When the pulp is first infected you may experience acute pain; over time, however, the pain may suddenly dissipate. This doesn’t mean the tooth has healed itself — quite the contrary, it may mean the infected pulp tissue, including the nerves, has died. Once the nerves die, they no longer transmit pain signals to the brain.
While the pain may cease, the infection hasn’t and will continue to travel from the end of the tooth root into the bone. At this point, you may encounter pain whenever you bite on the tooth. This time the pain is originating in nerves located in the periodontal ligament that surrounds the tooth root and joins the tooth with the jawbone. This can lead to an acute abscess (with accompanying pain) or a chronic abscess that may have no pain symptoms at all. As the decay progresses you may eventually suffer bone and tooth loss.
The important point here is that you may or may not notice all the signs and symptoms that indicate deep decay within a tooth. That’s why it’s important to undergo a thorough dental examination if you have any symptoms at all, especially acute pain that “mysteriously” disappears.
A root canal treatment and removal of the decayed tooth structure will stop the progress of tooth decay and preserve the tooth. The longer you delay, though, the greater the risk your tooth will eventually lose the battle with tooth decay and infection will continue to spread.
